About the Role
The Patient Navigator / Driver, Complex Care is a key member of Misiway’s Integrated Care Team. This role focuses on supporting clients—particularly those living with complex chronic conditions, serious illness, and life-limiting diagnoses—to access care through safe, reliable transportation and compassionate navigation support.
This position plays a critical role in reducing barriers to care by ensuring clients can attend medical appointments, access services, and return home safely. During transport and interactions, the Navigator provides practical, emotional, and culturally grounded support, helping clients feel comfortable, informed, and respected throughout their care journey.
Working within a wholistic model of care, this role incorporates a palliative approach by prioritizing comfort, dignity, and continuity of care, particularly for clients experiencing declining health or complex needs.
Grounded in cultural safety, trauma-informed practice, and Two-Eyed Seeing, this position brings together Indigenous and Western approaches to care with equal respect.
What You’ll Do
Transportation & Access to Care
• Provide safe, reliable transportation for clients to and from medical appointments, programs, and services within the community
• Assist clients in getting to and from appointments, including mobility support where appropriate
• Ensure client comfort and safety during transport, particularly for those with complex health needs
• Support appointment readiness (e.g., reminders, ensuring clients have required documents or medications)
• Help reduce missed appointments by coordinating schedules and maintaining clear communication with clients
Navigation & Client Support
• Support clients in understanding where they need to go and what to expect during appointments
• Provide companionship and reassurance during what can be stressful or overwhelming experiences
• Assist with basic system navigation, including wayfinding, appointment follow-ups, and connecting to services
• Communicate relevant client concerns or barriers to the care team to support continuity of care
Palliative Approach to Care
• Apply a palliative approach in day-to-day interactions by prioritizing comfort, dignity, and respect
• Provide supportive presence for clients living with serious illness, recognizing emotional, cultural, and spiritual needs
• Support access to care and services that enhance quality of life
• Honour Indigenous approaches to care, including facilitating access to Elders, ceremony, and Traditional supports as desired
Barrier Reduction & Wholistic Support
• Identify and help address barriers to care, including transportation, mobility, and access challenges
• Connect clients to community-based, cultural, and wellness supports as appropriate
• Support a welcoming and culturally safe experience for all clients
Documentation & Team Collaboration
• Maintain accurate documentation of transportation services, client interactions, and any relevant observations
• Communicate regularly with the care team regarding client needs, concerns, or changes observed
• Participate in team meetings as required
• Uphold client privacy and confidentiality in accordance with PHIPA and organizational policies
