Position Summary

Under the direction of the Quality, Risk, and Resident Experience Co-ordinator develop, apply and evaluate an individualized activation therapy program for residents independently or in collaboration with the clinical team.

Duties

• Develop and implement activities/programs to be conducive to residents with varying abilities and interests
• Assess the functional level of residents and assist residents as required to ensure full participation in programs
• Evaluate recreational activities/programs on an ongoing basis
• Continually assess and modify activation programs to meet the residents’ needs
• Develop a calendar reflecting on residents interests and needs
• Develop a monthly calendar to communicate activities/programs to assigned resident groups to participate in
• Document on visits, referrals, consults, and activation monthly
• Plan programs in conjunction with residents, recreation programmer, and the Recreation Therapist
• Act as an information resource for residents, families and staff regarding activation and therapies at the Golden Manor
• Support the care team in providing safe resident centered care within the scope of practice
• Complete any other duties assigned by Supervisor

Qualifications

• Degree or diploma in recreation and leisure studies, therapeutic recreation, kinesiology, gerontology or other related field
• Current experience in Long Term Care and knowledge of the aging process
• Able to work independently or collaboratively with the care team
• Good computer skills with knowledge of Microsoft Office Suite
• Familiarity with multimedia platforms to be utilized for communication and activities
• Gerontology experience is a definite asset
• Experience with the programs Point Click Care and Welbi is an asset
• Bilingualism (French and English) is a definite asset
• Current and clear Criminal Record Check with Vulnerable Sector Screen required
• Ability to adhere to all infection prevention and control protocols and screening requirements
