Description

The Behaviour Support Facilitator assists families living with dementia through in home support by providing tools, education and connections to help decrease challenging expressive situations caregivers may face. 

Travel within the district areas ( Kapuskasing to Temiskaming Shores)

Qualifications

·       Post-secondary education (diploma or undergraduate degree) in a health and/or social science program (e.g. Gerontology, Behaviour Science Technologist, Social Service Worker, Nursing, Allied Health,)
·       Prefer a minimum of 2 years’ experience working with older adults who have a   
neuro-cognitive disorder and/or mental health disorder and their families

·       Knowledge of community health care agencies
·       Excellent written and verbal communication skills
·       Demonstrated organizational skills
·       Valid driver’s license and access to a reliable vehicle
·       Experience in education delivery; current awareness regarding the effective use of best practice adult learning principles
·       Current CPR and First Aid Certificates

Core Competencies

·       Demonstrated knowledge and best practice in dementia, delirium, mental health disorders and the effect on persons, families, caregivers and system of care
·       Knowledge/Certification of U-FIRST; GPA; Montessori; P.I.E.C.E.S. will be given priority
·       Sound understanding of and commitment to the principles of the Behavioural Supports Ontario (BSO) project
·       Commitment to knowledge exchange as a means of continuous quality improvement,             education/coaching & capacity building in the individual, family and system of care
·       Demonstrated skills in data collection and program evaluation
·       Demonstrated effectiveness as a team member through collaboration, respect and effective communication skills in interacting with clients, family members, other members of inter-professional care teams and community partners in care
·       Demonstrated ability to adapt positively and productively to continuous changes in the work environment
·       Proficient verbal and written communication skills
·       Thorough knowledge of applicable community resources and how to navigate the health care system
·       Demonstrated knowledge in recovery/personhood philosophy
·       Ability to effectively deal with compassion fatigue Ability to meet physical and safety conditions
·       Proven good attendance record, organizing and prioritizing work effectively, emergency           management effectiveness
·       Computer skills
·       Valid driver’s license and access to a reliable vehicle with the ability to accommodate travel
·       Bilingualism is required(English/French)
